Leveraging Health IT Across a National Primary Care Practice Transformation Initiative: Lessons from EvidenceNOW

Conclusions: The current state of HIT implementation does not support the level of standardization necessary for practice transformation and population health initiatives in many small, primary care practices. Multi-stakeholder initiatives, like EvidenceNOW, have implications for other programs for taking data standardization to scale.
Fulfilling an ACA Promise: Infrastructure needed to scale up Primary Care Extension Programs

Conclusions: PCEPs have the potential to speed translation of research into practice. This study highlights infrastructure needed to take PCEPs to scale and demonstrates PCEPs' potential to facilitate integration of primary care and public health.
