What Do We Know and Need to Know About Transforming Primary Care?

As a long-time participant in, and student of, primary care transformation, I am struck by how little we understand about how to stimulate, facilitate, manage, and reward the major transformations we are seeking.

Leif Solberg, MD, Sr. Research Investigator, HealthPartners Institute and ESCALATES Dissemination Team Lead, opens the door for important questions, raises some questions for discussion, and hopes that others will be able to help provide evidence-based answers.

As a long-time participant in, and student of, primary care transformation, I am struck by how little we understand about how to stimulate, facilitate, manage, and reward the major transformations we are seeking. With hope that it might be helpful, my commentary published in Family Practice summarizes those experiences and the literature by identifying some of what we know, and still need to know, in order to guide and support care transformation. ESCALATES and the EvidenceNOW regional Cooperatives hope to provide at least partial answers for many of these questions. However, the more that those in other countries and contexts can help with this task, the more likely the answers will be useful for all the diversity of care settings.
